We found a significant 11% (8 to 14%) adjusted arVE favoring ccIV in preventing any IRO in the 2017вЂ“2018 influenza season. Nine publications with 22 results were at low risk of bias. Five publications reported 13 laboratory confirmed arVE and seven reported 13 code-ascertained arVE. We identified 12 publications that reported 26 adjusted arVE results. All estimates were computed using a random approach. We completed the literature search in January 2021 applied exclusion criteria, evaluated risk of bias of the evidence, and performed heterogeneity, publication bias, qualitative, quantitative and sensitivity analyses. We performed a systematic review and meta-analysis of the adjusted relative vaccine effectiveness (arVE) of seed cell-cultured influenza vaccines (ccIV) compared to egg-based influenza vaccines (eIV) in preventing laboratory-confirmed influenza related outcomes (IRO) or IRO by clinical codes, in subjects 18 and over. Extensive consideration of methodologic issues relating to rVE is needed, including the stability of estimates and the impact of confounding structure on the validity of rVE estimates.ДЄvian mutations in vaccine strains obtained from embryonated eggs could impair vaccine effectiveness. Assessment of twelve studies presenting both absolute and relative effect estimates suggested proportionality in the effects, resulting in implications for the interpretation of rVE effects.ДЄpproaches to rVE evaluation in practice is highly varied, with improvements in reporting required in many cases. Nearly two thirds of all studies controlled for age, and nearly half for comorbidities, region, and sex. Studies compared multiple vaccine components (n=38), two or more doses of the same vaccine (n=17), or vaccination timing or history (n=9), as one study compared a range of vaccine components and doses. We identified 63 articles assessing rVE in influenza virus. We extracted information on the study design, relative comparison made, and the definition and statistical approach used to estimate rVE in each study.

We screened titles, abstracts, full texts, and references to identify relevant articles. We conducted a systematic search on PubMed, Google Scholar, and Web of Science for studies reporting rVE comparing vaccine components, dose, or vaccination schedules. This review aims to identify how rVE evaluation is being performed in studies of influenza to inform a more standardized approach. When new vaccine components or platforms are developed, they will typically need to demonstrate non-inferiority or superiority over existing products, resulting in the assessment of relative vaccine effectiveness (rVE). The enhancing effect of prior infection versus the incrementally attenuating effect of prior vaccinations suggests that these exposures may alternately promote and constrain the generation of memory that can be recalled by a new vaccine strain. Titer rise was negligible among participants with 3вЂ“5 prior vaccinations, poor among participants with 1вЂ“2 prior vaccinations, and equivalent or better among those with prior infection compared to the reference group. Post-vaccination, titers were, on average, two-fold higher among participants with prior infection and two-fold lower among participants with 3вЂ“5 prior vaccinations compared to the reference group. Pre-vaccination, titers were lowest against 2009вЂ“2014 viruses among the reference (no recent exposure) group. Antibody responses were compared by fitting titer/titer-rise landscapes across strains, and by estimating titer ratios to the reference group of 2009вЂ“2018 viruses. The analysis was limited to participants aged 18вЂ“65 Y, and compared those exposed to A(H3N2) viruses circulating since 2009 by infection (Ha Nam) or vaccination (HCWs) to a reference group who had no recent A(H3N2) infection or vaccination (Ha Nam). Participants received licensed inactivated vaccines containing A/HongKong/4801/2014 (H3N2). Here, we combined these studies to directly compare antibody titers against 35 A(H3N2) viruses spanning 1968вЂ“2018. Analogously, we found that vaccine immunogenicity was enhanced by prior A(H3N2) virus infection among participants of the Ha Nam Cohort, Viet Nam, but was attenuated by prior vaccination among Australian Health Care Workers (HCWs) vaccinated in the same year. Prior vaccination can alternately enhance or attenuate influenza vaccine immunogenicity and effectiveness.
